***This form should be used for ALL Shabbos and Yomtov requests.***
For Weekday or Weekday+Shabbos requests use this form instead. (Accommodations during the week are for Married Shluchim Only - There is no weekday accommodation for Bochurim)
Requests:
- Please fill out the form for each person separately, and for each Shabbos/Yom Tov separately.
- Filling out the form does not provide an automatic confirmation. See Response time below.
- Reminder: we do not accept requests by email for Ohel Accommodations.
- Requests will be closed three days before Shabbos/Yom Tov begins.
Response time:
- Responses are usually provided within 36 hours before Shabbos/Yom Tov begins.
- (Note for Married Shluchim only: If you are requesting a bed for yourself and would like an earlier reply, please (1) fill out the form and then (2) email your request to ohel@shluchim.org so we can get back to you sooner. Include your name, shlichus location, and the Shabbos or Yom Tov you are requesting.)

(טופס זה מיועד לבקשות בשבת ויום טוב בלבד. עבור בקשות לימי חול (שלוחים נשואים בלבד) השתמשו בטופס זה.)
בקשות:
נא למלא טופס זה עבור כל אדם בנפרד, ועבור כל שבת/יום טוב בנפרד.
בקשות לשבת ויום טוב לא יתקבלו בדוא"ל.
הבקשות ייסגרו 36 שעות לפני כניסת השבת/יום טוב.

תגובות לבקשות בטופס זה יינתן אחרי 36 שעות לפני כניסת השבת/יום טוב.
(הערה לשלוחים נשואים בלבד: אם אתם מבקשים מיטה לעצמכם וברצונכם לקבל אישור מוקדם יותר, אנא (1) מלאו את הטופס ולאחר מכן (2) שלחו את בקשתכם בדוא"ל לכתובת ohel@shluchim.org כדי שנוכל לחזור אליכם מוקדם יותר. אנא כתוב את שמך, מקום השליחות ואת תאריך השבת או יום טוב שאתה מבקש ללון.)
